The following forms must be completed and submitted to the Head Scoutmaster in order to participate in Troop Activities:

- CORI Form (Criminal Offender Record Information)

To help you fill out the immunization record in Part A, here are the CDC abbreviation codes:
- Tetanus: TT, DT, DTaP, Td, Tdap
- Pertussis: DTaP, Tdap
- Diptheria: DT, DTaP, Td, Tdap
- Measles: MMR, MMRV
- Mumps: MMR, MMRV
- Rubella: MMR, MMRV
- Polio: IPV
- Chicken pox: Varicella, MMRV, VAR
- Hepatitis A: HepA
- Hepatitis B: HepB
- Meningitis: Menactra, MCV4, MPSV4
- Influenza: IIV, LAIV, LAIV4
